Initial Appointments: Most new patients require a physician referral to see one of our physicians. Your personal physician or surgeon will provide records of your medical condition and previous medical care. We also welcome the opportunity to provide second opinions. We accept most insurance plans and our staff will work with you to make sure that you have a thorough understanding of your coverage.
Please plan to arrive at our office at least 15 minutes prior to your scheduled time so that you can complete new patient forms, the receptionist can obtain any additional information that may be needed for your appointment, and can copy your insurance cards.
At every appointment: We do our very best to accommodate your scheduling needs. Please honor your scheduled appointment time. If you need to cancel an appointment, we request that you provide as much notice as possible.
