The Harbin Clinic contracts with various preferred provider organizations (PPOs), point of service (POS) plans, and health maintenance organizations (HMOs). Most Harbin Clinic physicians are considered participating providers under these plans. Insurance claims filed by most physicians in the Harbin Clinic with these carriers and managed care organizations will be considered in-network.

Medicare Advantage PFFS, HMO AND PPO PLANS

Private Fee for Service Plans (PFFS) - No contract is required. All Harbin physicians are considered participating physicians.

STATE HEALTH RETIREES: No contract is required for the State Health retiree plans with United Healthcare and Humana for 2011.  All Harbin physicians can treat these patients without penalty to the patient.
    
HMO/PPO - Contracts are required for these plans. Effective February 15, 2009 Harbin contracts with the following MC Advantage HMO/PPO plan(s).
